I hereby declare that:

• my sport parachute system is airworthy in the sense of the LuftVO and has been maintained according to the manufacturer's instructions;

• I will follow the instructions of the skydiving operations manager, the parachutist and the pilot;

• I am not under the influence of medication, alcohol or drugs;

• I am responsible for the airworthiness of my own skydiving equipment, but agree to a possible inspectio

• - I will only carry out activities for which I have the appropriate certificates of competence and valid authorizations;

• I am in possession of a valid skydiving license and the required liability insurance;

• I do not fly over the runway at an altitude of <600m/GND (2000ft);

• I adhere to the specified landing direction and do not perform any "turns" or turns over 90° to the landings;

• I agree to the minimum opening altitude of 800m/above ground level (2500ft);

• I have been briefed on the local conditions and have received a jump site briefing.

I expressly release the skydiving center operator, acting persons, Flyingbones Skydiving e.V. (as well as the above mentioned companies) and cooperating companies from any liability that could arise from the operation of my parachute, as well as damage to my person or property.

In the event of non-compliance with the instructions, the skydiving operations manager or board may exclude me from skydiving operations at any time.

I agree that videos and photos taken during my stay at the Flyingbones Skydiving e.V. jump site may be published on the homepage and print media of Flyingbones Skydiving e.V. and The Flyingbones UG and may be used for advertising purposes.
